Obituary of Forest W. Ballard

Forest W. Ballard, 68, a longtime resident of Puyallup, died on December 5, 2012 in Puyallup after a long battle was Parkinsons Disease. He was born on July 21, 1944 in Longview, WA. Forest was a longtime employee of Fiberboard (Sonoco) in Sumner and enjoyed fishing. He was a past Exalted Ruler of the Puyallup Elks. He was preceded in death by his parents and sister. Survived by his wife, Josie; Daughter, Teresa, Son, David (Melissa); Sister, Linda (Steve); Grandchildren, Chase, Madison, Dylan. Numerous nieces and nephews. A memorial service will be held on Sat. Dec. 29, at 2 p.m. at the Puyallup Elks, 314 27th St. N.E. Puyallup. In addition to flowers memorias may be made to the Michael J. Fox Parkinson Foundation, Grand Central Station, P.O.
